Terms and Conditions for CTL Website and CTL Application


1. Definitions

For the purpose of these Terms and Conditions, unless the context otherwise requires, the following terms shall have the meanings assigned to them below.

"Hospital" means CTL Hospital, including its management, medical personnel, employees, officers, representatives, departments, affiliated entities, service providers, and any authorized persons involved in the operation of CTL Website and CTL Application.

"Service" means any service provided through CTL Website or CTL Application, including but not limited to online appointment booking, doctor information, health check-up information, health articles, appointment notifications, online inquiries, contact forms, service packages, and any other digital services that may be added in the future.

"Appointment" means a booking or reservation made by a User to receive services from the Hospital, including doctor consultation, medical examination, health check-up, follow-up visit, test result consultation, or any other healthcare-related service.

"Health Information" means any information relating to a User's health, medical history, symptoms, treatment, medication, appointment, examination, test result, medical service, or any other healthcare-related data.

"Personal Data" means any information relating to a person that can identify such person, whether directly or indirectly, including name, surname, telephone number, email address, identification number, date of birth, address, device information, system usage data, and Health Information.

"Sensitive Personal Data" means Personal Data that is specifically protected by applicable law, including Health Information, medical records, biometric data, or any other sensitive data under data protection laws.

"Cookies" means small data files stored on a User's device when accessing the Website or Application, which help the System remember user preferences, improve performance, analyze usage, and enhance user experience.

"External Service Provider" means any third party, company, vendor, technology provider, cloud service provider, payment service provider, SMS provider, email service provider, analytics provider, marketing technology provider, or other service provider engaged by the Hospital to support the operation of the System or related services.


2. Scope of These Terms and Conditions

The User agrees to use the System personally and shall not transfer, assign, lend, lease, share, or allow any other person to use their User Account without authorization from the Hospital.

The User is responsible for maintaining the confidentiality of username, password, OTP, PIN, device access, or any other authentication information.

If the User becomes aware of unauthorized use of their account or any security concern, the User should notify the Hospital as soon as possible so that the Hospital may investigate, suspend, or take appropriate action.


4. Registration and Information Provided by Users

Users who wish to access certain services through CTL Website or CTL Application may be required to register or provide information as specified by the Hospital.

Information required for registration or service use may include:

4. Date of birth

5. Gender

The User represents and warrants that all information provided to the Hospital is accurate, complete, current, and not misleading.

If the User's information changes, the User should update such information to ensure that the Hospital can provide services, contact the User, send notifications, and manage appointments accurately.

The Hospital reserves the right to reject, suspend, restrict, or terminate a User Account if the Hospital finds or reasonably suspects that the information provided is false, inaccurate, incomplete, outdated, misleading, or may cause damage to the Hospital or any other person.


5. Identity Verification and Account Security

For the safety of Users and the protection of Personal Data, the Hospital may require identity verification before granting access to certain services.

Online appointment services may include:

An appointment shall be considered confirmed only when the User receives confirmation from the System, the Hospital's staff, or any confirmation channel designated by the Hospital.

The date, time, doctor, department, service details, or other information displayed in the System is provided for convenience and may be subject to change due to operational necessity.

The Hospital reserves the right to reschedule, cancel, modify, or change appointment details where necessary, including cases where the doctor is unavailable, the schedule is full, there is a medical emergency, there is a system error, or there is any other reasonable cause.

The Hospital will use reasonable efforts to notify the User of any changes through appropriate channels.

Users should arrive at the Hospital before the scheduled appointment time as instructed by the Hospital in order to complete registration, eligibility verification, payment, medical preparation, or any other necessary procedures.

If the User is unable to attend the appointment, the User should cancel or reschedule in advance through the System or through the Hospital's contact channels.


7. Appointment Information and Service Timing

The User acknowledges that online appointment booking is intended to facilitate reservation and queue management only. It does not guarantee that the User will receive service exactly at the scheduled time in all cases.

Actual service time may depend on several factors, including:

1. Number of patients on the service date

2. Emergency cases

3. Duration of consultation for each patient

4. Availability of doctors and medical personnel

5. Availability of examination rooms

6. Availability of medical equipment

7. Completeness of patient information

8. Insurance or eligibility verification

9. Technical or operational issues

10. Force majeure events

The Hospital will use reasonable efforts to provide services according to appointment schedules but does not guarantee that all services will be provided at the exact scheduled time.


8. Appointment Cancellation and Rescheduling

Users may cancel or reschedule appointments through the channels and conditions specified by the Hospital.

The Hospital may set advance notice requirements for cancellation or rescheduling, such as at least 24 hours before the appointment time, or any other period depending on the type of service.

If a User fails to attend an appointment without prior notice, the Hospital may record such non-attendance for queue management, service improvement, and future appointment consideration.

For services that require advance payment, deposit, reservation fee, or package purchase, cancellation, rescheduling, and refund shall be subject to the specific terms applicable to each service.

The User acknowledges that certain payment services may be subject to the terms, conditions, processing time, security standards, and policies of the relevant External Service Provider.

The Hospital shall issue receipts, payment confirmations, or transaction documents in the format and manner determined by the Hospital.


12. Refund and Cancellation of Paid Services

21. User Obligations and Prohibited Conduct

Users agree to use CTL Website and CTL Application lawfully, honestly, appropriately, and in accordance with these Terms and Conditions.

Users shall not:

1. Use the System for unlawful purposes

2. Provide false, inaccurate, or misleading information

3. Use another person's information without authorization

7. Use the System to harass, deceive, threaten, or harm others

11. Use automated tools, bots, scraping tools, or data extraction tools without permission

12. Perform any act that the Hospital considers inappropriate or potentially harmful

If the Hospital finds or reasonably suspects that a User has violated these Terms and Conditions, the Hospital may suspend, restrict, terminate, or take other appropriate action against the User Account.


22. Suspension, Restriction, or Termination of User Account

The Hospital may suspend, restrict, or terminate a User Account if any of the following occurs:

8. Reliance on Website or Application content without consulting a doctor

9. Third-party websites, applications, or services

This limitation of liability shall apply to the fullest extent permitted by applicable law.


26. Health Information and Medical Disclaimer

Health Information provided on CTL Website or CTL Application is for general educational and informational purposes only.

Diagnosis, treatment, prescription, medical procedure, or medical decision must be conducted by a licensed physician or qualified healthcare professional.

Users acknowledge that health conditions differ from person to person, and general information may not be suitable for the specific condition of each individual.

If the User has severe symptoms, emergency conditions, or uncertainty regarding their health, the User should seek direct medical attention immediately.

The Application may offer a preliminary disease risk-assessment feature (e.g., for diabetes, dyslipidemia, and stroke) powered by an artificial intelligence (AI) system developed and operated solely by the Hospital. This system processes the health information entered by the User to generate a preliminary risk estimate only.

The risk-assessment result generated by this AI system does not constitute a medical diagnosis, treatment, or personalized medical advice, and does not replace consultation with, or diagnosis by, a licensed physician under any circumstances. Users should consult a doctor for an accurate diagnosis and advice appropriate to their individual health condition.

Health information entered by the User for this risk-assessment feature is not shared with any external AI provider or third party, and is not retained after the assessment result is generated.


27. Complaints, Feedback, and Contact

Users may contact the Hospital to inquire, report issues, file complaints, or provide feedback regarding CTL Website, CTL Application, or related services.

Upon receiving a complaint or inquiry, the Hospital will review the matter as appropriate and may request additional information from the User to support investigation or resolution.

The Hospital will use reasonable efforts to respond or resolve issues, but response time may depend on the complexity of the matter, type of issue, required verification, and departments involved.


28. Service Modification, Suspension, or Discontinuation

The Hospital reserves the right to modify, improve, suspend, discontinue, replace, or remove any part of CTL Website, CTL Application, or related services at any time.
